Workshop in Weissensee

From the 16. to 19. january 2025 the 20th SEF Workshop carried out.

This annual meeting of the SEF Scientific Advisory Board will once again take place at the Seehotel Enzian and the Weissensee Haus.

The conference is organized and chaired by Prof. J. Keckstein and Prof. M. Sillem. All members of the Advisory Board are invited to attend, whereby active participation is expected for the entire duration of the conference. The Board of Directors may also invite other persons whose expertise it considers helpful for the meeting.
A working group will deal with the sensible combination of surgery and reproductive medicine. It has proven successful when tandems participate that also work together in daily clinical routine.

Workshop in Weissensee

From February 10 to 13, 2024, the 19th SEF workshop was held. was held.

For organizational and scheduling reasons, the SEF Scientific Advisory Board had to hold this workshop from Saturday to Tuesday.
The conference was organized and chaired by Prof. J. Keckstein and Prof. M. Sillem.

Workshop in Weissensee

From 19 to 22 January 2023, the 18th SEF workshop was held. was held.

Unfortunately, the conference could only take place with a limited number of participants due to spatial and organizational limitations. Only active members of the scientific advisory board were invited.
The working groups below were divided up in advance in order to have 10 participants per group for the relevant topics. Your own ideas on the topic should be presented in a short statement.

The conference was chaired by Prof. Dr. med. J. Keckstein and Prof. Dr. med. K.-W. Schweppe (standing in for Dr. med. K. Bühler, who was absent due to illness).

The three topics currently on the agenda were addressed:

1. which quality criteria are checked and how?
The results summarized by Prof. Dr. M. Sillem and Prof. Dr. I. Meinhold Heerlein can be found here as a basis for discussion for the future.

2. Europeanization of certification
The considerations and concepts summarized by Prof. Dr. H. Tinneberg and Prof. Dr. K.-W. Schweppe can be found here, whereby the individual points must be coordinated and defined with the relevant partners.

3. interdisciplinary imaging diagnostics:
#Enzian from a radiological perspective (MRI findings, training, guidelines) and #Enzian from an ultrasound perspective (more differentiated imaging, further development of #Enzian specifically for ultrasound?).
You can read the results summarized by Dr. P. Klein and Dr. J. Metzler here; they are the basis for further coordination with the US companies and the radiologists.

Workshop in Weissensee

From 20 to 23 January 2022, the 17th SEF Workshop was held.

Despite the worsening coronavirus situation, a small group met in person and held lively discussions in two working groups to address the issues at hand. On Saturday afternoon, the results were presented to the plenary session, with colleagues who were unable to attend due to the coronavirus also taking part in the lively discussion via ZOOM.

The conference took place in the premises of the Hotel Enzian (incl. Seehaus) under the direction of Prof. J. Keckstein and Prof. M. Sillem.
The three specified topics are dealt with:

3. certification criteria

Workshop in Weissensee

The 15th annual conference of the SEF took place from February 14 – 17, 2019 at the “Weissensee-Haus” in Neusach/Carinthia.

This year there was once again a record number of participants with >90 endometriosis experts from German-speaking countries. In addition to topics relating to diagnostics and drug and surgical therapy, issues relating to follow-up therapy, pain therapy, documentation, certification and classification were also discussed in detail. As in previous years, the program consisted of a balanced mix of scientific presentations and small group work in working groups (WG). On the last day, the results of the working groups were presented and discussed in plenary. The summaries of the working groups are listed below and can be downloaded.

MOMENDO workshop in Münster

As part of the EU research project MOMENDO, which deals with the molecular mechanisms of endometriosis, the Women’s Clinic is organizing an international workshop on ‘microRNAs in Endometriosis’ in Münster on 09/10.02.2017. (see below: info flyer).
Another workshop will take place in Edinburgh on September 20, 2017 under the direction of Prof. Philippa Saunders, Prof. Andrew Horne and Dr. Erin Greaves. (see info flyer below).

From the February 15 – 18, 2018, the 14th annual conference of the SEF took place at the “Weissensee-Haus” in Neusach/Carinthia.

Many participants had prepared a short presentation, a case report or a surgical video on one of the topics of the working groups as an “entry ticket”, so to speak, for the 14th workshop – similar to last year.

Workshop in Weissensee

This year’s Weissensee Conference took place from February 16 – 19, 2017 at the “Weissensee-Haus” in Neusach/Carinthia.
Many of the more than 60 participants (the capacity limit was reached!) had prepared a short presentation, a case report or a surgical video on one of the conference topics as an “admission ticket”, so to speak, for the 13th workshop.

The program consisted of numerous short presentations with sufficient time for partly controversial discussions and the meetings of the working groups. (see download below).

This year’s workshop took place from February 18 to 21, 2016 at the “Weissensee-Haus” in Techendorf 78, 9762 Weissensee.
Under the proven leadership of Prim Prof. Dr. J. Keckstein, the participants worked diligently and engaged in lively discussions from Thursday afternoon until Sunday afternoon. The participants heard presentations with sufficient time for discussion in the plenary session from:

As always, the main work was carried out in the working groups, which continued to develop ideas and theses well into the evening.

Under the direction of Prim. Univ. Prof. Dr. J. Keckstein, the conference was a complete success.
Due to the bad weather, the lunch break was canceled, so that from morning to evening there were concentrated, sometimes controversial and scientifically sound discussions. A special feature was the demonstrations on the various pelvic specimens from the anatomy department of the University of Munich!
The capacity limit was reached with over 40 participants – unfortunately some interested parties did not get a place!

Endometriosis remains an enigmatic disease in women of reproductive age. Current treatment regimes used to manage the disease are primarily designed to induce a hypoestrogenic state which leads to a reduction in disease and associated symptoms. While these regimes have been used with great success, there are still drawbacks and limitations to these types of therapies. In this review, we address the shortcomings of current drugs used to medically manage endometriosis and offer potential target areas which may be attractive alternatives to current therapies. Emphasis is placed upon the promising research using TNF inhibitors in this context. Further, we discuss the rationale and current status for medical therapy of endometriosis with aromatase inhibitors, SERMs and COX 2 inhibitors.

The syndrome of dislocated endometrium (SDE)

Today, there is no doubt that transtubar dissemination of desquamated endometrium is the “cause” of pelvic endometriosis. Subfertility and sterility are major side effects of endometriosis. However, the complete surgical or medical removal of endometriosis lesions does not solve the infertility problem. The question therefore arose as to whether sterility in pelvic endometriosis is a direct consequence of the lesions, which must be doubted after the above explanation, or whether endometriosis is a much more complex clinical picture with the lesions and sterility as consequences of a hitherto unidentified underlying disorder.
Retrograde menstruation is a physiological phenomenon, although there is no data on its quantity. Endometriosis, on the other hand, only affects a small, albeit considerable, proportion of women of reproductive age.
Apart from “physiological retrograde menstruation”, however, there is a scientifically established retrograde transport function of the uterus that is of fundamental importance in the early process of reproduction. This is the cervico-fundal uterine peristalsis for directed sperm transport and high-fundal implantation of the embryo. Our research approach to clarifying the pathophysiology of endometriosis was therefore based on the question of whether there is a relationship between the development of endometriosis and a disruption of this physiological transport function of the uterus.
Archimetra and Neometra
Investigations into the morphological and functional basis of this transport function led to the concept of Archimetra. The archimetra is a phylogentically, ontogenetically and functionally independent organ within the uterus. It comprises the subendometrial musculature, the stratum subvasculare or archimyometrium with its circular fiber orientation, and the endometrium with all its three layers, the basalis, spongiosa and functionalis. The characteristic feature of the archimetra is its paramesonephric origin and the control of its function by the ovary. The archimetra is phylogentically very old and becomes ontogentically recognizable as a primordial uterus in the second trimester after fusion of the Müllerian ducts.
In contrast, the outer muscle layers of the uterus, the vascular and supravascular stratum, are not of paramesonephric origin but develop from the mesenchyme of the uterine serosa in the third trimester or even postpartum. In phylogenesis, these muscle layers only develop after the appearance of viviparity. In reference to the term Archimetra and in view of its ontogenetically and phylogenetically late development, we have chosen the term Neometra for this outer part of the uterus. The neometra is under the control of the conceptus, at least in primates.
The main functions of the archimetra are the preparation of the endometrium for implantation (proliferation and secretory transformation), the directed transport of sperm and the prevention of infection based on the immunological competence of the endometrium (macrophages; MUC-1; leukocytes). The neometra has the function of providing the expulsion forces sub partu.
The archimetra as the site of development of endometriosis
Studies on the function and structure of the archimetra in women with endometriosis have led to the view that the archimetra and in particular the basal endometrium play a central role in the development of endometriosis. The following phenomena support this concept.
Hyper- and dysperistalsis of the archimyometrium
In infertile women with endometriosis, there is increased peristaltic activity throughout the cycle, which is on average 100% higher than normal in the early and middle proliferation phase. As a result, these women already have a significantly increased retrograde transport capacity of the uterus during the menstrual phase. At the same time, the internal uterine pressure is increased.
Desquamation of basal endometrium
During menstruation, the functionalis is shed and the functionalis regenerates from the basalis or the area of the spongiosa close to the basalis. In 80% of women with endometriosis, we were able to detect fragments of basalis in the menstrual blood, whereas this was only the case in 10% of women without endometriosis. Due to the considerably increased retrograde transport capacity of the uterus in these women, transtubar fragments of basalis enter the peritoneal cavity in increased numbers, where they implant due to their genuine proliferative potency and possibly grow infiltratively and form endometriosis foci.
The parallel development of adenomyosis
Using transvaginal sonography (TVS) and magnetic resonance imaging (MRI), we were able to demonstrate that endometriosis in infertile women is associated with adenomyosis with a prevalence of 90%. Adenomyosis leads to destruction of the structure of the archimyometrium, which is responsible for sperm transport, and is therefore at least partially responsible for sterility in women with endometriosis.
The cause of the functional and morphological changes of the archimetra in women with endometriosis is a paracrine hyperestrogenism of the basalis. Throughout the cycle, the basalis expresses P450 aromatase, which forms estrogens from androgenic precursors. As the basal layer in women with endometriosis is twice as thick as in healthy women, more oestrogen is produced locally at the level of the archimetra, which has a paracrine effect. With identical peripheral oestradiol levels in the blood of women with and without endometriosis, those in the menstrual blood of women with endometriosis are significantly increased. This may be due solely to the thickening of the basalis but also to increased cellular production.
As an interim result in the study of endometriosis, the following conclusion can therefore be drawn
Endometriosis is based on a paracrine interference of increased basal estrogens at the level of the archimetra with the ovarian control of uterine peristalsis. This results in hyperperistalsis with the dislocation of basal cell fragments – transtubar retrograde with the consequence of peritoneal endometriosis and subbasal infiltrative with the development of adenomyosis.

The 2nd Workshop of the Scientific Endometriosis Foundation took place from January 24 – 26, 2003 at the Seehotel Enzian. The first Enzian classification of deep infiltrating endometriosis was developed here as a supplement to the rASRM classification.

The 1st working conference of the SEF took place from January 18 – 20, 2002 at the Seehotel Enzian.

Organized by Prim. Prof. Dr. med. J. Keckstein and under the direction of him and Prof. Dr. med. K.-W. Schweppe, 12 motivated endometriosis specialists from German-speaking countries met to discuss clinical issues relating to the surgical and medical treatment of the disease and, above all, the divergent literature data on surgery for deep infiltrating intestinal endometriosis. Taking into account their own experiences, they came to the conclusion that a classification of deep infiltrating endometriosis should be developed so that surgical results and problems can be analyzed and compared clinically and scientifically.
